If you're considering discussing starting a dosage higher than typically suggested, there are certain circumstances under which this may be allowed:

  • If you have been on the same GLP-1 agonist medication, either at the requested dose or one titration level lower, for at least a month, and you haven't experienced any side effects.

  • If you're currently taking Tirzepatide or Semaglutide and wish to switch medications

    • If you've received care outside of Maximus, you'll need to fulfill the same conditions listed above.

To help your healthcare provider make the best decision for your health, you should provide some documentation from your previous provider. The necessary documents would include:

  • Recent prescription (with your name & DOB) – upload via the client portal

  • Medication bottle photo (showing name, date, and dosage)

  • Doctor’s note with recent weight & prescribed dose

  • Clinical notes from your previous provider, including recent dose & weight

What if my current dose isn't available in your system?

Can I select a different dose if my exact dosage isn't listed? Yes! If you don't see a dosage that matches what you've previously been on (for example, 11.25mg of Tirzepatide weekly), you can still proceed with your application.

How to Select Your Dose:

  • Always select the closest available dose without rounding up (always round down to the nearest available option)

  • Example: If you're currently on 11.25mg Tirzepatide, select the closest lower dose available in our system which is 10mg Tirzepatide.

Important Steps:

  • Communicate your actual dose: Be sure to let your Maximus provider know in any communication which exact dose you're currently taking

  • Provide documentation: We may ask for supporting documentation (such as prescriptions or clinical notes) so we can facilitate your current dosing accurately

  • Discuss adjustments: If, after discussing with your Maximus provider, you decide to move up to the next available dose, we'll be happy to accommodate you
