Login to Elation Passport Account (iOS app)
Click the Pencil & Note icon (top right)
Click 'Select Reason'
Choose 'Lab or Other Test Results'
Click Paperclip Icon (Add Attachment)
Either 'Take A Photo OR Choose From Library'
