Three ways to complete blood tests
At-home test kit — A Maximus at-home kit covers all required blood markers for your protocol.
Quest Diagnostics lab draw — A Maximus-provided requisition lets you complete an in-lab blood draw at any Quest Diagnostics location.
External bloodwork — You can submit recent bloodwork from another provider (LabCorp, Quest Diagnostics, or others), as long as it meets Maximus criteria.
Your clinician determines which option is appropriate based on your protocol.
Ordering a blood test through Maximus
Request the test through the Maximus platform.
Specify any additional markers you'd like included (for example, Hematocrit or DHT).
You'll be notified of the final panel and any additional charges before the test is ordered.

The standard Maximus blood panel typically includes:

If you want markers not covered in the standard panel:
For markers like ALT and AST (not included in at-home kits): use a standard laboratory requisition for an in-lab draw at a national lab. This lets you include these markers alongside your ordered labs. Maximus typically provides the lab requisition form within 24 hours after payment confirmation.
For markers Maximus doesn't offer at all (such as blood thickness or other specialized tests): you'll need an order from your primary care physician (PCP).
As an alternative, you can submit external bloodwork that includes the markers you need.
Scheduling a Quest Diagnostics blood draw
To schedule a blood draw at Quest Diagnostics:
Bring the Quest Diagnostics lab requisition (provided by Maximus) and a valid photo ID to any Quest Diagnostics location.
Use Quest's location finder and booking page to select a site and schedule your appointment.
At the appointment, present the lab requisition and your photo ID. Quest will perform the blood draw.
The lab coordination team cannot schedule appointments on your behalf — you must book directly with Quest.
After completing your test, upload or share the result file in your Maximus message thread so your care coordinator can add the results to your chart for review.
Submitting external bloodwork
Maximus accepts external bloodwork completed within the last 6 months, provided it includes all required markers for your protocol.
