Skipping Testing: If you decide not to proceed with testing, you will not be automatically billed or sent an at-home lab kit. You can choose to skip providing a lab sample and decide later if you want to complete the labs.
Charges for Lab Kits: You are not charged for every lab kit by default. For example, after completing the required follow-up tests for Testosterone Replacement Therapy, the next scheduled lab is typically the annual lab. Additional charges only apply if a doctor requests an extra test or if you proactively order one. Lab test charges are separate from your subscription fee. Your subscription covers access to the program; individual lab tests are billed independently when applicable.
Quest Labs Cost: If you choose Quest Labs for your blood tests, the cost is $115. Maximus provides a requisition form for you to bring to any Quest Labs location. Note that out-of-pocket fees apply for Quest Labs.

How to book your draw
After your clinician orders your labs, you'll receive an email from Maximus with your Quest requisition (a PDF) and a link to find a Quest Patient Service Center near you.
Walk-ins are accepted at most locations, but appointments are recommended and can be booked at QuestDiagnostics.com.
Bring a photo ID and your requisition (digital on your phone is fine — no printing required).
Fast for 8–12 hours before your draw if your requisition says "fasting required." If you're unsure, default to fasting in the morning with water only.
When you'll get results
Quest sends results to Maximus within 2–3 business days of the draw. Your clinician reviews them within 1–2 business days after that. You'll receive an email when results are reviewed and your protocol is updated.
If your draw can't be completed
