Lab Testing: Costs and Policies
Maximus provides flexible options for lab testing. This article covers costs, policies, and how to manage your lab testing needs.
Lab testing options
There are two ways to complete your required labs:
Maximus at-home test kit ($99): A single kit covers the full marker panel required for your chosen protocol. The kit cost covers sample collection and processing — there is no additional fee. Kits are delivered within 2–3 business days. Collect the sample at home and return it using the prepaid label.
Bring Your Own Labs (BYOL): Upload bloodwork from your own provider or clinic. Labs must be within the last 6 months and include all required markers for your protocol. This option is free if you already have qualifying labs.
Quest Diagnostics (in-person draw)
Quest is an in-person blood draw option for customers who prefer not to use the at-home kit, or whose protocol requires tests the at-home kit doesn't support (full hormone panels, metabolic panels, lipid panels, etc.).
What it costs
Quest draw costs vary by panel and range from $19.99 to $115. A lab requisition form is provided — bring it to any Quest Diagnostics location. If a Quest location asks you for payment at the time of your draw, decline and contact Maximus support — we will resolve it directly with Quest.
How to book your draw
After your clinician orders your labs, you'll receive an email from Maximus with your Quest requisition (a PDF) and a link to find a Quest Patient Service Center near you.
Walk-ins are accepted at most locations, but appointments are recommended and can be booked at QuestDiagnostics.com.
Bring a photo ID and your requisition (digital on your phone is fine — no printing required).
Fast for 8–12 hours before your draw if your requisition says "fasting required." If you're unsure, default to fasting in the morning with water only.
When you'll get results
Quest sends results to Maximus within 2–3 business days of the draw. Your clinician reviews them within 1–2 business days after that. You'll receive an email when results are reviewed and your protocol is updated.
If your draw can't be completed
If Quest is unable to complete your draw (vein access issues, equipment problems, location closed), no charge applies and you can rebook at any Quest location using the same requisition. The requisition is valid for 30 days.
Quest vs. at-home kit
Use the at-home kit when your protocol only requires testosterone (total + free) and you prefer to test at home. Use Quest when your protocol requires a fuller panel, when the at-home kit didn't yield enough sample, or when your clinician specifies an in-person draw.
Required markers by protocol
Enclomiphene only: Total Testosterone, Free Testosterone or SHBG, LH, FSH
Oral, Topical, or Injectable Testosterone: Total Testosterone, Free Testosterone or SHBG, LH, FSH, PSA, Hematocrit
Labs must be within the last 6 months to be accepted.
Testing frequency
Baseline lab before starting treatment
Follow-up lab at 1 month after starting
Follow-up lab at 3 months after starting
Annual lab at 12 months and each year thereafter, unless your clinician directs otherwise
Additional labs may be requested after side effects, dose changes, or protocol switches
Replacement kits
If a replacement kit is needed due to a damaged, lost, or malfunctioning kit, you will not be charged for the replacement. The replacement will include the same marker panel as the original.
Managing your lab preferences
Automatic kits: If your account is set to the at-home kit option, you'll be automatically charged and sent a kit at each required testing interval.
Switching to BYOL: Contact support to switch. Once switched, you manage lab submissions through your own provider and manually request lab orders through your dashboard.
Skipping testing: You will not be automatically charged or sent a kit if you choose to skip. You won't be charged if you skip, but your protocol renewal may be affected if labs are overdue.
FAQs
Are follow-up labs included in my subscription? No. Follow-up labs are a separate charge. They are automatically ordered at the required intervals.
How much do follow-up labs cost? Follow-up at-home kits are $99. Quest draw costs vary by panel ($19.99–$115).
What if I don't have a nearby Quest location? Use the at-home test kit instead.
Do I need to stop taking testosterone before my follow-up lab? No. Complete labs as instructed while continuing your protocol unless your clinician advises otherwise.
Will my replacement kit include the same tests? Yes. Replacement kits include the same marker panel as the original, including SHBG and free testosterone if applicable.
Can I avoid automatic charges? Yes. Switch to BYOL to manage labs through your own provider.
