Foundayo (orforglipron) is Eli Lilly's once-daily oral pill for weight management, FDA-approved in April 2026. It's the first oral non-peptide GLP-1 receptor agonist on the market — meaning it's a different class of molecule than Wegovy, Zepbound, or any prior GLP-1 medication.
How Foundayo is different from other GLP-1s
All GLP-1 receptor agonists work by activating the same receptor, but Foundayo gets there differently. Wegovy, Ozempic, Zepbound, and Mounjaro are peptide molecules — they mimic naturally occurring hormones and must be injected or, in the case of Wegovy oral tablets, taken with a special absorption enhancer on a strict empty stomach. Foundayo is a small molecule — a simpler chemical structure that can be absorbed directly through the gut with or without food, and without any fasting window.
Practical difference: Foundayo is taken once daily, with or without food, no waiting period. Wegovy oral tablets require an empty stomach, up to 4 oz of plain water, and a 30-minute fast before eating or taking other medications.
How to take Foundayo
Take one tablet by mouth once daily. Swallow whole — do not break, crush, or chew. You can take it with or without food. No fasting window, no water restriction, no waiting period before eating or other medications.
Dosing and titration
Foundayo titrates through six steps, with at least 28 days at each step:
0.8 mg → 2.5 mg → 5.5 mg → 9 mg → 14.5 mg → 17.2 mg
Important: Every patient starts at 0.8 mg — including patients already on semaglutide, tirzepatide, or any other GLP-1. Prior GLP-1 history does not carry over. Foundayo binds the GLP-1 receptor differently, so the body needs the same titration period to adjust regardless of experience with other medications.
What to expect from Foundayo
From published clinical trials:
Foundayo 17.2 mg: ~11.1% mean body weight reduction at 72 weeks
Wegovy 2.4 mg injection: ~14.9% mean body weight reduction at 68 weeks
Zepbound 15 mg: ~20.9% mean body weight reduction at 72 weeks
These are from different trials in different patient populations and are not direct comparisons. In general, published data places Foundayo's top dose below Wegovy's standard maintenance dose for weight reduction. Patients who expect Wegovy- or Zepbound-level results from Foundayo may be disappointed. Your provider can help set realistic expectations.
Side effects
The most common side effects are gastrointestinal — nausea, constipation, diarrhea, and vomiting — consistent with the GLP-1 class. Some patients in trials also reported fatigue, headache, and hair loss. Most side effects ease as the body adjusts during titration.
Drug interactions
Foundayo has more drug-interaction considerations than injectable GLP-1s. Important interactions:
Strong CYP3A4 inhibitors (some antifungals, certain antibiotics, certain antivirals) — cap Foundayo's max dose at 9 mg
Strong CYP3A4 inducers — should be avoided entirely with Foundayo
Simvastatin — should not exceed 20 mg daily when taken with Foundayo
Oral hormonal contraceptives — require backup contraception for 30 days after starting Foundayo and after each dose escalation

Switching to or from Foundayo
Coming from another GLP-1: You will restart at Foundayo's lowest dose (0.8 mg) and titrate up through the full ladder. No exceptions based on prior GLP-1 history.
Switching from Foundayo to an injectable: You'll restart at the injectable's lowest dose and titrate up through that product's standard schedule.

Can I skip the starting dose since I'm already on a GLP-1? No. Every patient starts Foundayo at 0.8 mg regardless of prior GLP-1 history. This is not a Maximus policy — it's how the medication works clinically.
