You may be approved to use your own lab/blood work test if the following criteria are met:

• The lab panel must include Total Testosterone.

• The lab panel must also include SHBG and/or Free Testosterone.

• The sample must have been collected within the last six months.

• The sample must have been collected at least two weeks after discontinuation of the following: Anabolic Steroids, SARMs, SERMS, or Aromatase Inhibitors.

You must also agree to use the same lab vendor and panel for your follow-up lab test (completed about 30 days after starting treatment, or as required by your licensed Maximus clinician).
