Maximus uses identity and location verification to ensure your care is safe, legally compliant, and provided by a clinician licensed in your state. These steps are required after you’ve chosen what protocol you’d like to engage with or submitted for a test kit, and will be completed before treatment can begin.
Why Identity Verification Is Required
Telehealth regulations require clinicians to confirm the identity of every patient they treat. Verification ensures:
Safe and accurate medical records
Prevention of fraud or misuse
Compliance with state and federal laws
Your information is encrypted and used only for medical compliance.
How Identity Verification Works
You will be asked to upload a valid government-issued ID. The verification system checks:
Whether the ID is authentic
Whether your photo matches your intake
Whether your information is complete
If verification cannot be completed, you may be asked to upload a clearer photo or alternative identification.
ID verification may fail if:
The image is too dark or blurry
The ID is expired or damaged
Information on the ID does not match the intake
The name or birthdate was entered incorrectly
If this happens, simply re-upload your ID or follow the instructions provided by Support.
Why Location Verification Is Required
Your clinician must be licensed in the state where you are physically located at the time of treatment. Location verification ensures:
Safe and legal prescribing
Correct clinician assignment
Accurate protocol availability
Location verification may involve:
Address confirmation
Secure digital verification
Validation that your state matches the clinician’s licensure
This step also ensures that prescriptions are delivered to the correct address.
Updating Your Address
If you move or change addresses:
Update your information in the patient portal
You may be asked to re-complete location verification
Your protocol options may change based on your new state
Keeping your address current ensures accurate pharmacy delivery. Prescriptions cannot ship until your address is verified.
What Happens If Verification Fails?
Medications must ship to your verified address. If identity verification fails:
You’ll receive a notification
You may be asked to retry or upload additional information
Support may reach out if anything requires clarification, or you can reach out to the team directly if you encounter an issue.
Do I Need to Re-Verify If I Move?
Yes. If you move to a new state, re-verification is required because your clinician must be licensed in that state.
Your available protocols may change depending on state regulations.
