Your intake questionnaire is the first step in starting any Maximus protocol. A licensed clinician uses your answers to evaluate whether treatment is medically appropriate, decide what labs you need, and choose the right starting protocol. Until your questionnaire is complete and reviewed, no medication can be prescribed or shipped.
WHAT EACH SECTION COVERS
Symptoms: What you're experiencing and how long. Be specific β "low energy for 6 months" is more useful than "tired."
Medical history: Past and current conditions, surgeries, hospitalizations. Include even items you think are unrelated.
Current medications and supplements: All prescriptions, OTC drugs, and supplements. This is checked for interactions before any prescription is issued.
Allergies: Drug, food, and environmental. Critical for prescription safety.
Lifestyle factors: Sleep, stress, diet, exercise, alcohol, and substance use. These influence treatment choice and dosing.
Treatment goals: What you want from the protocol. This helps your clinician choose between options (e.g., topical vs. injectable testosterone).
WHY HONESTY AND COMPLETENESS MATTER
Incomplete or inaccurate answers can cause: prescription delays (clinician requests follow-up before approving), an inappropriate protocol being chosen, or treatment denial. You can update your questionnaire at any time through your dashboard if your situation changes.
WHAT BLOCKS SHIPMENT
Your medication will not ship until ALL of the following are complete:
Intake questionnaire submitted
Required labs completed (if your protocol requires baseline bloodwork)
Clinician review approved
Shipping address verified
Payment method on file
If your shipment is delayed, the most common cause is an incomplete questionnaire or a clinician follow-up question still open in your dashboard.
COMMON QUESTIONNAIRE ISSUES AND FIXES
"I can't get past a section." The questionnaire blocks progress until required fields are answered. Scroll up to find any field marked with a red dot or "required" label.
"I'm not sure how to answer a medical history question." Answer to the best of your knowledge. Your clinician will follow up if anything needs clarification.
"I made a mistake and already submitted." Log into your dashboard and request an update. Your clinician will see the new answer before approving treatment.
"My clinician asked a follow-up question." Open your dashboard inbox β clinician follow-ups appear there, and treatment is paused until you respond.
"It's been more than 3 business days and I haven't heard back." Contact support and we'll confirm whether the questionnaire was received and whether anything is
pending.
